Mr Bates is definitely a tortured soul, who has perhaps found a soulmate in Anna. His past however, keeps coming back to haunt him. He is reminded of his problems by the physical pain he suffers as well as the mental trauma and frustration that he is caused by having to harbour too many secrets, especially from Anna.
